Hi, my name is Jacobus and I am the owner/service provider of Acorn Disability Services. Originally from South Africa, I have a passion for people, meeting individuals and getting to know them. My extensive client services experience will ensure that only the very best care is provided.

Acorn Disability Services is an owner operated support provider, ensuring client centered service. I operate on a one to one basis, with no middle man. Your contact person at Acorn Disability Services is the person who will be providing care, book appointments, provide assistance and who will be there each step of the way, based on the care required.

Acorn Disability Services offers assistance to Plan Managed and Private managed participants.

Areas serviced are Toowoomba, Gatton, Oakey, Dalby, Pittsworth and all surrounding areas.

We proudly support all members of the community, including the LGBTQIA+ community.

Acorn Disability Services is an NDIS registered Service Provider. Rates charged are in line with the current NDIS pricing guide, ensuring fair fee management. Each client will receive a Service Agreement and Schedule of support, as well as a Participant Handbook. An invoice is provided after each shift.

I am available for any morning, afternoon, evening, late night shifts or even sleepovers if required. I also operate on Saturdays, Sundays and Public Holidays. Please call, send an email or use the message facility at the bottom of the web page, and I will get back to you.

Assist with personal activities

Assistance is provided with playing games, going for walks, reading to a participant, doing any hobbies, organizing rooms/shelves, etc

Assist with Travel/Transport

Transport can be arranged for: doctors appointments, meetings, work, any other appointments, sporting events, restaurants, movies, etc

Daily Tasks/Shared Living

Assistance provided includes meal preparation, helping with showering/bathing, getting dressed, personal hygiene (clipping of nails, brushing hair, shaving, toileting) and any other assistance required.

Household Tasks

Assisting with cleaning house, vacuuming, washing floors, wiping counters, doing laundry, doing dishes, cleaning of shelves and fridges, and any other household tasks.

Participation in Community activities

Assisting with activities outside the home, i.e meeting with friends, going on coffee dates, doing grocery shopping, going to the library, going to physio/hydro sessions, going to the movies, volunteering at an organization, going to church.

Group activities

Sporting events, group outings with other individuals, going to classes, and any activity where you meet up with other individuals to perform a recreational activity, for instance bowling leagues.
